Dear Ph.D. Student:

It's time for full-time and part-time students to complete the annual Ph.D. review  - see attached documents.

Please connect with your research advisor to complete your form asap! Dr. Pavel Skums is your temporary assigned advisor if you do not have a research advisor yet.

Please note: If you are finishing your 4th year and you are supported by the department,  please submit a statement (up to 2 pages) including your research achievements and teaching activities. The Grad Committee will evaluate and decide whether to continue your support for the 5th year.
